On context model for mvp_flag and ref_idx

Abstract
This contribution reports a mismatch between VTM-7.0 and VVC working draft 7 related to the coding of mvp_l0_flag, mvp_l1_flag, ref_idx_l0, and ref_idx_l1. In VTM-7.0, mvp_l0_flag and mvp_l1_flag are coded with a single context. While in WD7, these two syntaxes are coded with separate contexts. Same issue was detected for ref_idx_l0 and ref_idx_l1 and reported in ticket #749. This contribution investigated two methods for solving this issue: 1) software is aligned to spec and 2) spec is aligned to software.
