CE4-related: Simplified coding of the MVP index

Abstract
This contribution proposes to simplify the coding of the motion vector prediction index in AMVP mode. This syntax element, called mvp_l0_flag and mvp_l1_flag in JVET-M1001, is context-based coded in VTM-4.0. This contribution proposes to code it in bypass mode.
