AHG8: On output layer set and PTL signaling

Abstract
In this contribution, signaling output layer set that indicate the operation points of multi-layered bitstreams with profile/tier/level information in VPS is proposed. Similar to SHVC, profile_tier_level_idx[ i ][ j ] is signaled in VPS, specifies the index, into the list of profile_tier_level( ) syntax structures in the VPS, of the profile_tier_level( ) syntax structure that applies to the j-th layer of the i-th OLS. An additional minor modification is that vps_output_layers_mode[ i ] is signaled for each output layer set. In V2, the derivation process of the values of direct or indirect dependent layer indices from vps_direct_dependency_flag is added. With the derivation process, layer_included_flag for each output layer set may not be necessary.
