Non-CE4: Simplification of HMVP in AMVP mode

Abstract
This contribution proposes to simplify the HMVP insertion process in AMVP mode. One loop in the HMVP insertion process is removed, and POC check is replaced by reference index check. The Y, U, V BD-rate changes under common test conditions are 0.00%, 0.01%, -0.01% in RA and 0.01%, 0.06%, -0.21% in LDB. Similar performance is observed in the low QP test, the Y, U, V BD-rate change are 0.00%, -0.03%, 0.03% in RA and 0.00%, 0.04%, -0.03% in LDB.
